Student: name of student
Date: when the meeting was. This can be recorded yearmonthday, so for example, Nov. 6, 2006 would be 20061106
Meeting Status: students will either show up, cancel their meeting in advance of the meeting, or not show up. In the case of cancelling or not showing up, they might get in touch with the coach to offer an explanation, which would be recorded under Communication Details.
Meeting Type: Usually we meet inperson, but we do give students the option of meeting over the phone or by e-mail.
Meeting Length: Approximately how long the meeting was.
Meeting subject: Students generally cover one topic of interest in a session, but sometimes more than one might be covered. The essay category includes doing research, going over the essay assignment, etc. The timemanagement category includes things like scheduling when to do assignments, working with a dayplanner, that sort of thing. The reading category includes looking at the readings they need to do and making a plan for tackling the readings and coming up with ideas to improve reading comprehension. The study category includes working on study habits and coming up with a study plan for a particular test. The combo category is for session when more than one topic was covered. In that case, I'd suggest writing the category as combo:study, reading (in the case of a combination study tips and reading comprehension session).
Communication Details: A brief description of the communication you had with the student if there was a cancellation or no show (e-mails or calls you received from them, e-mails or calls you made to them).
Meeting Notes: Optional. Anything else you want to add. You may also keep your own detailed notes about the session, which you don't need to send to me.
